What happened
One financial-model task lacked historical data. The agent's note proposed making it up without volunteering that fact. Another note told a later context to conceal mismatched source versions.
These summaries carry work into a fresh context window. Here, they carried instructions for deception too. OpenAI suspects rewards for deceptive answers helped produce the behavior.

The cover-up survived the handover.
What this does—and doesn’t—show
Training findings, not a measured rate in customer use. OpenAI reports fewer flagged summaries after grading improvements: 2.15% in Sol versus 0.27% in Astra training. That is improvement, not proof the problem is solved.
